ARTICLE (1): SABIC CORPORATE BASIC POLICY

SABIC provides healthcare for its Saudi employees and their legal dependants, as well as for its Non-Saudi contracted employees and their legal dependants residing with them and have regular legal residence authorization (Iqama) according to the labor contract at the medical facilities specified or agreed to by the company.

ARTICLE (2): EMPLOYEE MEDICAL EXAMINATIONS

The company reserves the right to ask the employee to undergo medical examination at any time during the term of employment. The employee is required to agree to do such examination.

ARTICLE (3): MEDICAL EXPENSES REIMBURSMENT

3-1 Reimbursement of the expenses of medical emergency and non- emergency cases in approved medical facilities in KSA

SABIC will cover (100%) of the medical expenses incurred by the employee in company assigned, contracted or approved medical facilities via Any medical insurance company (medical card); and the employee shall not pay in cash for the covered services. If the employee chooses to pay in cash, the company shall incur 100% of the cost according to the agreed rates with the same medical facility.

3-2 Reimbursement of medical expenses in non-approved medical facilities in Kingdom of Saudi Arabia
Emergency cases:
If the employee receives medical treatment in non-assigned medical facilities in KSA, SABIC will bear (100%) of the cost of medications, subject to the highest rate in KSA medical network.
Non-emergency cases:
If the employee receives medical treatment in non-assigned medical facilities in KSA, SABIC will incur (80%) of the medical treatment and other services expenses, and (100%) of the cost of medications, subject to the highest rate in KSA medical network.
3-3 Reimbursement of medical expenses in emergency cases outside Kingdom of Saudi Arabia

In emergency cases involving immediate medical care such as accidents, heart attacks and cases if delayed may lead to death or may affect any vital body function, the employee and his dependents – who are legible for medical care during vacations or business trips - may see the nearest doctor or hospital. The Company shall pay (100%) of the costs of treatment and medication (obstetrics not included). Dependents without their family provider shall be covered for (90) days maximum per dependant per year, according to the above controls.

3-4 Reimbursement of medical expenses in non-emergency cases outside Kingdom of Saudi Arabia

Non-emergency treatment or surgeries abroad during vacations and business trips are subject to Company previous approval. (80%) of the costs shall be reimbursed, subject to highest rates in the network of medical facilities in KSA. Supporting documents, including a detailed report of the medical need for treatment and medications issued, are required.

3-5 Reimbursement of medical expenses of visiting physician

After paying the visiting physician in cash, reimbursement shall be subject to highest rate in KSA medical network.

3-6 Reimbursement shall be subject to the controls of benefits and exceptions, as explained in this program.

ARTICLE(4): CONTROLS OF MEDICAL TREATMENT

4-1 Rules for medical treatment of Saudi and Non-Saudi employee’s children
A. Saudi’s children:

The company will continue to provide the health care coverage for Saudi’s male children until such time that they are employed or they reach the age of (24) years whichever occurs first. The Company will continue to provide health care coverage for females until they get married or are employed whichever occurs first.

B. Non-Saudi’s Children

The company will cover a maximum of (4) four children not exceeding the age of (18) years, provided that they legally reside in K.S.A.

C. New born babies shall be covered by their mother’s card for (60) days as of the date of birth.

4-2 Rules for medical care for employee’s parents

The employee should provide full, or partial, dependency document issued by the religious court.

4-3 Controls of medical care after demise or total disability

Medical care coverage shall be extended to the legal dependents of the Saudi employee deceased, as well as the totally disabled and his legal dependents, for a period of five years effective from the date of demise or total disability.

4-4 Controls of covering maternity/ obstetrical care for the employee’s mother

Medical care shall be covered for the employee’s mother, but not the new born baby from the minute of the birth.

ARTICLE(5): MEDICAL REFERRAL

5-1 Medical referral is subject to the consideration of the medical committee formed by the Company for studying weather the case needs referral or not, according to applied medical terms.
5-2 Referral is subject to non-availability of the treatment in the area.
5-3 Controls of referral to specialized hospitals inside KSA

If the case the patient requires specialized treatment not provided in Company assigned facilities, the patient may be referred to a specialized hospital inside KSA at company expense. For referral, a medical report form a Company approved facility is required.

5-4 Controls of referral to specialized hospitals abroad

The Company will bear the cost of treatment outside K.S.A. for employee/dependant, this needs a Medical Report from assigned medical facility - or two reports from Company approved medical facilities - showing that the illness or treatment cannot be treated in K.S.A. and the treatment procedure is not available as well. The Medical Report should include the following :-

1-Type of Disease/Sickness
2. Reasons of inability of in-Kingdom treatment.
3. Suggested country where the treatment will take place.
4. The necessity of companion with the Patient.
5. Expected period/duration of treatment.
6. Any other necessary information the company may ask for to take decision.

ARTICLE(6): SERVICES RELATED TO HEALTH CARE IN THE EVENT OF REFERRAL TO HOSPITALS INSIDE OR OUTSIDE THE KINGDOM

A) Transportation:

The Company will cover the costs of transportation in economy class if the place of treatment is outside the geographical area of the working place, this includes the costs of a companion according to a recommending Medical Report. The company will consider the transportation class based on the severity of the patient’s situation, as per Medical Report.

B) Room, Board and Transportation inside the city of the treatment for the Patient and his Companion:

The Company will cover the actual expenses for Room, Board and Transportation for both the Patient and his Companion; if it is proven that his case requires a companion as follows :-

In-Kingdom :

The Company shall pay SAR (650) to the patient and his companion.

Out-of-Kingdom :

The Company shall pay SAR (1000) to the patient and his companion.

ARTICLE(7): COVERED MEDICAL SERVICES

7-1 The following medical equipment are covered:

1-Optical appliances, including lenses and frames (total coverage SAR 1000). Maximum coverage for the frame is (SAR 500) per person per year, when needed.

2-Artificial limps: to be issued once per year per person, when needed.

3-Medical supplies: IV solutions, bandages, pharmaceuticals, syringes, glucose strips, lancet, pen needle, alcohol swab for diabetics, crutches, leg varicose socks, neck disc collar and medical brace when needed and described by the doctor.

4-Hearing aids: Maximum SAR 4000 per person per year.

5-Steam devices: Maximum SAR 750 per person every three years.

6-Sugar measuring devices: maximum SAR 500 per person every three years.

7-Wheel chair: When medically needed up to SAR 600.

8-Speech Therapy : up to the age of seven years old and for medically indicated cases of post surgical & post CVA during admission period.

ARTICLE(8): MEDICAL EXPENSES EXCLUDED FROM COVERAGE

The Company shall not incur the following medical expenses, unless were due to work injury :-

1- Investigations and treatment for infertility and impotence, all methods of birth control, artificial insemination and in vitro fertilization, and all other methods of assisting conception.

2- Correction of refraction errors by laser or lasik, keratotomy, corneal graft and muscle surgery, squint operation for children above the age of seven, sun and safety glasses and contact lenses (corneal full thickness grafting is covered where no other options to restore vision are available).

3- Physical fitness, weight loss and obesity treatment.

4- Dental: Dental bridges, crowns, temporary or permanent partial/complete dentures, dental implantation, orthodontics, dental bleaching, fluoride, fissure sealants, toothpaste, dental floss or brushes.

5- Medical checkup more than once a year.

6- Special custodial care, home nursing and non-functional rehabilitation.

7- Acupuncture, hypnotism, herbal treatment, osteopathy and trial treatment.

8- Infant milk products and food supplements (except during period of hospitalization).

9- Treatment with vitamins and hormones, unless as a therapy for illness of deficiency, as shown by tests.

10- Treatment of baldness and adolescence acne (unless acute and inflamed, to be covered with antibiotics) and cosmetics.

11- Medical equipment and devises(unless covered or needed for surgery), medical shoes and medical beds.

12- Vaccines related to allergy treatment and smoking cessation programs.

13- Mental regardless, suicide attempts (unless du to psychological diseases) or injuries caused by dangerous activities.

14- All cases resulting from alcoholism, use of drugs or any other hallucinatory substances.

15- Plastic and cosmetic surgeries, except for post accidental injury and post surgical reconstruction surgery.

16- any test and/or treatment not required by a medical physician or any pharmaceutical products not licensed by or not registered at the Ministry of Health.

17- Diagnostic investigations related to driving license issuance and renewal; and travel check up, if not for business trip.

ARTICLE(9): REHABILITATION OF DISABLED CHILDREN

The Company shoulders (80%) of the costs incurred by the employee for the rehabilitation of his disabled children including costs of transportations in accordance with the following controls :-

1- Rehabilitation shall be carried out in the Kingdom in a center acceptable to the Company.

2- Lack of free rehabilitation facilities.

3- The employee is not receiving any government support or support from any other entity.

4- A medical report acceptable to the company indicating the type of disability and the required rehabilitation.

5- Approval of the competent authority.

ARTICLE(10):MEDICAL COVERAGE FOR RETIRED SAUDI EMPLOYEE UNDER REGULAR RETIREMENT POLICY (60) YEARS

The Company shall incur the value of medical insurance, according to its approved policies, at an approved insurance company for any retired Saudi employee under regular retirement policy, his wife/wives and his parents, if he was their dependant before retirement, according to the following controls : -

1- He must have completed (10) years, minimum, of service at the Company.

2- The period of the medical coverage must not exceed (5) years of the date of retirement.

3- He should not practice any other job that entitles him to medical care.
